Description
Centered in a posterior orientation, the heart is shown with the left atrium forming much of the base, flanked by the right atrium and the superior and inferior venae cavae entering on the right side. The left and right ventricles sit inferiorly, with the interventricular groove and posterior (diaphragmatic) surfaces providing surface landmarks for underlying myocardium and coronary venous drainage. Superior to the ventricles, the pulmonary trunk and right and left pulmonary arteries course laterally toward the hila, while pulmonary veins return medially to the left atrium; the pericardium frames the cardiac silhouette. Color coding distinguishes arterial and venous channels, from the great vessels down to arterioles and capillary beds. Posterior anatomy matters because many clinically relevant structures are easiest to conceptualize from this side, starting with the pulmonary veins and their direct entry into the left atrium, the chamber targeted in atrial fibrillation ablation and influenced by left atrial enlargement from mitral valve disease. The oblique pericardial sinus and reflections around the great vessels also make more sense in posterior view, a useful mental map when interpreting pericardial effusion patterns or planning posterior pericardiotomy. Short and direct: this is where the pulmonary venous return meets the heart. In cardiothoracic surgery teaching, this angle supports discussions of pulmonary venous anatomy, left atrial access, and the relationship of the heart’s base to the lungs during procedures such as lung transplantation or left atrial appendage management. It also fits radiology and anatomy curricula when correlating a posterior cardiac silhouette with CT angiography or MR views of pulmonary arteries and veins.
