Rhomboid Muscles

Description

Centered on the posterior neck and upper thorax of an adult male, the rhomboid minor and rhomboid major are highlighted deep to the superficial dorsal musculature, spanning from the vertebral column to the medial border of each scapula. Rhomboid minor arises from the nuchal ligament and spinous processes of C7 to T1 and inserts near the root of the scapular spine, while rhomboid major runs inferiorly from the spinous processes of T2 to T5 to the medial scapular border below the spine. The paired muscles sit medial to the scapulae and lateral to the cervical and thoracic spinous processes, with adjacent bony landmarks including the scapular spine, medial border, and posterior ribs. Rhomboid anatomy matters whenever scapular position is part of the problem. Weakness from dorsal scapular nerve injury (C5), often related to traction or entrapment as the nerve passes through the middle scalene, produces impaired scapular retraction and subtle winging that differs from the more dramatic medial winging seen with long thoracic nerve palsy affecting serratus anterior. Palpation and manual testing rely on these attachments: you bias the rhomboids by asking for scapular adduction and slight downward rotation, then compare left to right at the medial border.
