Description
Posterior thoracic anatomy is presented with the rhomboid minor and rhomboid major highlighted deep to the superficial dorsal tissues, spanning from the spinous processes of the lower cervical and upper thoracic vertebrae to the medial (vertebral) border of the scapula. Rhomboid minor lies superior to rhomboid major, inserting near the root of the scapular spine, while rhomboid major extends inferiorly toward the scapular inferior angle along the medial border. The vertebral column remains midline, with paired scapulae positioned laterally over the posterior ribs and the occipital region visible superiorly. Rhomboids matter when you need to explain scapulothoracic mechanics and why “scapular winging” is not a single diagnosis. Weakness or denervation of rhomboid major and minor, classically from dorsal scapular nerve injury (C5) during traction, repetitive overhead activity, or iatrogenic injury during posterior cervical or scapular procedures, produces impaired scapular retraction and subtle lateral displacement that contrasts with serratus anterior palsy. Palpating and testing these muscles also depends on knowing their relationship to the medial scapular border and the thoracic spinous processes. Small muscles, big consequences.
