Description
Centered on the posterior thoracic wall, the rhomboid major muscles are shown bilaterally spanning from the spinous processes of the upper thoracic vertebrae to the medial (vertebral) borders of the scapulae. Each muscle runs obliquely inferolaterally, lying deep to trapezius and positioned inferior to the smaller rhomboid minor near the C7 to T1 level. Laterally, the scapular spine and acromion frame the superior shoulder girdle, while the ribs form a curved lattice lateral to the vertebral column. Bony landmarks are crisp. Rhomboid anatomy matters any time scapular position drives symptoms. In scapular dyskinesis, overlengthened or inhibited rhomboids permit excessive scapular protraction and anterior tilt, patterns often seen with subacromial pain and poor overhead mechanics. The medial scapular border is also a frequent pain generator in myofascial syndromes, and this posterior perspective clarifies where rhomboid trigger points refer pain and how they differ from levator scapulae or mid trapezius tenderness.
