Description
Seen in right lateral profile with the skin rendered semi-transparent, the risorius (Santorini) is highlighted as a thin, variably developed slip coursing superficially across the cheek. Its fibers run roughly horizontally from the parotid and masseteric fascia overlying the masseter, traveling anteriorly to blend at the modiolus at the angle of the mouth, where it interlaces with orbicularis oris and the depressor anguli oris. Deep to it, the masseter occupies the lateral mandibular ramus, while the zygomatic arch and body of the mandible provide the bony contour. Fascia matters here. Clinically, this lateral, subcutaneous view clarifies why the risorius is an unreliable landmark in facial dissection and why its apparent “origin” can shift between individuals, often arising from superficial musculoaponeurotic system (SMAS) and parotid-masseteric fascia rather than bone. That variability becomes relevant in rhytidectomy and cheek procedures, where dissection planes near the SMAS and parotid region must respect the nearby buccal branches of the facial nerve and the parotid (Stensen) duct as they course across the masseter before turning medially. The risorius also helps explain the mechanics of a transverse smile or grimace by retracting the mouth corner laterally.
