Description
Positioned on the posteromedial thigh, the semimembranosus appears as a broad, flat hamstring muscle originating from the ischial tuberosity and descending deep to the long head of biceps femoris and lateral to the gracilis. A distinct intermuscular groove separates it from the more superficial semitendinosus, with the adductor magnus lying anterior and medial along the femur. Superiorly, the muscle belly relates to the posterior hip region beneath the pelvis, while distally its tendon courses toward the medial tibial condyle, posterior to the knee joint capsule. Bony context includes the lower lumbar vertebrae, pelvic girdle, proximal femur, and the knee-level relationship of femur to tibia and fibula. Clear landmarks. Clinically, this separation plane between semimembranosus and adjacent hamstring components is the corridor you follow when localizing hamstring strains and partial tears on exam and MRI, since semimembranosus injuries often occur proximally at the ischial origin or at the musculotendinous junction and can mimic adductor pain. The distal tendon’s contribution to the posteromedial knee, including its expansion toward the oblique popliteal ligament, also matters when teaching posteromedial corner anatomy and differentiating tendinopathy from meniscal pathology. Palpation and image interpretation both depend on understanding which structure is medial, which is superficial, and where the tendons converge.
