Description
Running along the posteromedial thigh, the semimembranosus muscle is presented with its broad, flattened proximal belly and a stout distal tendon that lies deep to the semitendinosus. Proximally, its fibers arise from the ischial tuberosity and course inferiorly toward the knee, positioned medial to the biceps femoris and posterior to the adductor magnus. Near the popliteal fossa, the tendon narrows and descends to the posteromedial tibia, with the medial femoral condyle and tibial plateau forming the bony backdrop. This medial orientation also frames adjacent regions of the hip, knee, and leg musculature, including the gluteal mass superiorly and the posterior compartment of the calf inferiorly. Clinically, this view clarifies why semimembranosus pathology can be mistaken for medial meniscal or pes anserinus pain, since its distal tendon and expansions sit close to the medial joint line and the medial collateral ligament region. The semimembranosus contributes to knee flexion and tibial internal rotation, and its tendon gives a reflected expansion that blends with the posterior capsule, the oblique popliteal ligament, and the fascia over popliteus, a key teaching point when discussing posteromedial corner stability and arthroscopy landmarks. Hamstring strain patterns also make more sense here. Deep tendon, broad origin.
