Description
Running along the posteromedial thigh, the semitendinosus appears as a fusiform, straplike hamstring with a long distal tendon that courses inferiorly toward the medial aspect of the knee. Proximally, its muscle belly lies posterior to the femur and medial to the biceps femoris, with the semimembranosus positioned deeper and slightly more medial. Distally, the semitendinosus tendon becomes superficial as it passes posterior to the medial femoral condyle and approaches the anteromedial proximal tibia as part of the pes anserinus, adjacent to the sartorius and gracilis tendons. Fiber direction and tendon contrast are clear. Medial viewing angles matter for teaching both compartment anatomy and palpable landmarks around the medial knee. The semitendinosus tendon is commonly harvested as an autograft for anterior cruciate ligament reconstruction, and the medial perspective helps clarify its relationship to the superficial medial collateral ligament and the pes anserine bursa, where bursitis can mimic medial meniscal pain. It also helps distinguish hamstring strains at the musculotendinous junction from pathology involving the deeper semimembranosus tendon near the posteromedial corner. Orthopaedic and sports medicine texts use this configuration to illustrate hamstring anatomy, pes anserinus layering, and graft-harvest planning, while gross anatomy and kinesiology courses often pair it with surface anatomy of the medial knee and the mechanics of knee flexion and tibial internal rotation.
