Description
Running along the posteromedial thigh, the semitendinosus is presented as a superficial hamstring muscle lying posterior to the femur and medial to the biceps femoris, with the broader semimembranosus positioned deep to it. Proximally, its fibers arise from the ischial tuberosity of the pelvis, then taper into a long distal tendon that courses inferiorly toward the medial aspect of the knee. From this relationship, you also appreciate how the hamstring compartment relates to the hip joint superiorly and to the tibia and fibula at the level of the knee. Clinically, the semitendinosus matters because its long, palpable tendon is a common autograft source for anterior cruciate ligament reconstruction, often harvested with the gracilis through an anteromedial tibial incision at the pes anserinus. That same medial insertion helps explain its role in resisting anterior tibial translation and in controlling internal rotation of the tibia during gait. A frequent pain generator. Strains typically occur proximally near the ischial tuberosity or at the musculotendinous junction during sprinting and kicking, and this superficial over deep arrangement helps differentiate semitendinosus involvement from deeper semimembranosus injury on examination and imaging.
