Semitendinosus Muscle

Description

Running along the posterior thigh, the semitendinosus muscle is rendered from its proximal attachment on the ischial tuberosity to its long, narrow distal tendon as it courses toward the medial aspect of the knee. Adjacent hamstring anatomy is implied by context, with the semimembranosus lying deeper and broader on the posteromedial thigh and the biceps femoris positioned more laterally toward the fibular side. Superiorly, the pelvis is shown with the iliac crest and acetabulum framing the proximal femur, while distally the femoral condyles lead into the knee region where the semitendinosus tendon becomes distinctly cord-like. Posterior relationships read cleanly. This posterior perspective matters because the semitendinosus tendon is a common autograft source for anterior cruciate ligament (ACL) reconstruction, and surgical harvest depends on recognizing its superficial position relative to the semimembranosus and its trajectory toward the pes anserinus on the anteromedial tibia. Small errors here have consequences. Confusing the tendon with the gracilis, or straying too proximally near the popliteal fossa, can complicate dissection and increase the risk of iatrogenic injury to the saphenous nerve and its infrapatellar branch, a well-known cause of postoperative numbness around the anteromedial knee.
