Viewed from a superior perspective into the opened cranial cavity, the trigeminal nerve (CN V) is traced from its root entry zone at the anterolateral pons to the gasserian (trigeminal) ganglion within Meckel’s cave on the petrous part of the temporal bone. From the ganglion, the ophthalmic (V1), maxillary (V2), and mandibular (V3) divisions course anteriorly and diverge toward their respective foramina at the skull base, with V1 directed toward the superior orbital fissure, V2 toward the foramen rotundum, and V3 toward the foramen ovale. Surrounding bony landmarks include the frontal bone anteriorly, the maxilla with dentition inferiorly, and the floor of the middle cranial fossa laterally and posteriorly, where grooves and ridges help orient the nerve pathways. Clear spatial relationships. An overhead intracranial view like this earns its keep when teaching the topography that underlies trigeminal neuralgia and skull base procedures, because you can follow the sensory root to the trigeminal ganglion and then watch each division separate in a way that matches symptoms and targets.
