Centered on the floor of the middle cranial fossa, the trigeminal (Gasserian) ganglion lies in Meckel’s cave on the anterior surface of the petrous temporal bone, just lateral to the dorsum sellae and cavernous sinus region. From this semilunar ganglion, the ophthalmic nerve (V1) courses anterosuperiorly toward the superior orbital fissure, the maxillary nerve (V2) runs anteriorly to the foramen rotundum, and the mandibular nerve (V3) descends toward the foramen ovale. A superior, intracranial perspective also places the trigeminal root proximally at the lateral pons and shows how the foramina cluster at the sphenoid and temporal bones. Clear bony landmarks. Surgical planning around the trigeminal system depends on these spatial relationships because the ganglion and divisions sit at the crossroads of skull base corridors and common pain procedures. Percutaneous rhizotomy, balloon compression, and radiofrequency ablation for trigeminal neuralgia target the ganglion via the foramen ovale, so the trajectory relative to the petrous ridge, clivus, and adjacent vascular structures matters. This view also helps teach why cavernous sinus pathology can affect V1 and V2 (lateral wall) while lesions near foramen ovale preferentially involve V3, with corresponding sensory loss and weakness of the muscles of mastication. Neuroanatomy and head and neck anatomy courses use this plate to anchor the trigeminal pathways to real foraminal anatomy, and it reproduces well in atlases and exam review material when discussing cranial nerve exit points and skull base approaches.
