Description
Arising from the tympanic plexus on the promontory of the middle ear, the lesser petrosal nerve is traced anterosuperiorly across the petrous part of the temporal bone toward the floor of the middle cranial fossa, where it runs medial to the greater wing of the sphenoid and approaches the foramen ovale. Nearby, the mandibular division of the trigeminal nerve occupies the same exit corridor, while the greater petrosal nerve courses more anteromedially toward the region of the foramen lacerum. From a superior view of the cranial base, the petrous ridge separates the middle cranial fossa from the posterior fossa, and the relevant landmarks include the internal acoustic meatus, jugular foramen, and the bony margins of the sphenoid foramina. Following the lesser petrosal nerve matters because it carries preganglionic parasympathetic fibers from the glossopharyngeal nerve (CN IX) to the otic ganglion, then postganglionic fibers travel with the auriculotemporal nerve to the parotid gland. The anatomy becomes clinically concrete when evaluating gustatory sweating (Frey syndrome) after parotidectomy or when planning skull base approaches that traverse the infratemporal fossa, where injury to the otic ganglion pathway can alter parotid secretion. Small, but unforgiving. Educators can use this superior cranial base perspective to teach the parasympathetic outflow of CN IX alongside foraminal anatomy in head and neck, neuroanatomy, or gross anatomy labs, and authors will find it well suited for atlas plates covering cranial nerve pathways, middle cranial fossa corridors, and otic ganglion connections.
