Description
Arising from the tympanic plexus on the promontory of the middle ear, the lesser petrosal nerve is shown coursing anterosuperiorly across the petrous part of the temporal bone toward the anterior surface of the cranial base. Along its path, the nerve relates medially to the region of the internal acoustic meatus and more anteriorly approaches the area of the foramen ovale, where it typically exits the middle cranial fossa to reach the infratemporal fossa. Yellow cranial nerve elements contrast against the bony skull base, allowing the lesser petrosal nerve to be tracked in relation to neighboring foramina and adjacent trigeminal pathways. Clinically, this is the preganglionic parasympathetic route from the glossopharyngeal nerve (CN IX) to the otic ganglion, and its anatomy is easiest to teach when the skull base landmarks are explicit. Injury or entrapment along the petrous temporal surface or near the foramen ovale can interrupt secretomotor fibers destined for the parotid gland via the auriculotemporal nerve, producing reduced parotid salivation. Otologic disease involving the middle ear mucosa can also affect the tympanic nerve and plexus, making the proximal origin of the lesser petrosal nerve a useful correlation for symptom-based teaching. Faculty can drop this into neuroanatomy and head and neck anatomy modules when covering parasympathetic ganglia, CN IX branches, and skull base foramina, and it also supports surgical teaching for middle cranial fossa and infratemporal fossa approaches where the foramen ovale is a working corridor.
