Description
Superficial to the dorsal tarsus, the extensor hallucis brevis belly lies anterolateral to the talus and calcaneus, arising from the superolateral calcaneus and inferior extensor retinaculum region and running distally toward the hallux. Its tendon courses medialward across the dorsum to join the dorsal expansion at the base of the proximal phalanx of the great toe, adjacent to the extensor hallucis longus tendon, which remains more central and continues to the distal phalanx. Lateral to this hallux slip, extensor digitorum brevis tendons fan toward toes 2 to 4 over the metatarsals, with the cuneiforms and metatarsal shafts faintly visible as bony landmarks beneath the musculotendinous layer. Skin has been removed, but the relationships remain those of a true dorsal foot dissection. Dorsal foot extensor anatomy matters when pain localizes to the anterolateral ankle and dorsum, where extensor tendons can be irritated by shoe pressure, overuse, or synovitis around the tendon sheaths. Clear separation of extensor hallucis brevis from extensor hallucis longus helps when teaching hallux extension testing and when interpreting dorsal forefoot ultrasound or MRI, since a slip from extensor hallucis brevis can be mistaken for tendon splitting or a partial tear if the normal course is not recognized. Small muscle, frequent confusion. Ideal for lower limb anatomy labs, podiatry and orthopaedic teaching files, and surgical education on dorsal approaches to the midfoot and first ray where extensor tendons and the inferior extensor retinaculum guide safe planes of dissection. Also well suited to textbook figures on gait mechanics and hallux function, where distinguishing intrinsic from extrinsic extensors improves clarity.
