Description
Seen from the dorsum of a male foot, the extensor hallucis brevis is presented as a short intrinsic muscle belly on the superolateral aspect of the midfoot, with its slender tendon running distally toward the hallux. Deep bony landmarks of the tarsus and metatarsus are visible beneath the soft tissues, including the talus and calcaneus proximally, the navicular and cuboid in the midfoot, and the cuneiforms articulating with the metatarsals. The tendon courses anterior to the tarsometatarsal region and approaches the proximal phalanx of the great toe, where it contributes to extension at the first metatarsophalangeal joint. Joint surfaces are tinted to indicate the synovial articulations across the tarsal, tarsometatarsal, and metatarsophalangeal levels. Dorsal foot anatomy can look deceptively spare, so isolating extensor hallucis brevis helps clarify how hallux extension is shared between extensor hallucis longus and the intrinsic extensor mechanism.
