Description
Superficial skin is rendered semi-transparently over the dorsum of a male foot, allowing the extensor digitorum brevis muscle belly to be appreciated as a low, broad mass on the dorsolateral forefoot. From its origin on the superolateral calcaneus, fibers run distally and anteriorly, giving rise to short tendons that course medial to lateral toward the lesser toes to join the long extensor tendons over the metatarsals and proximal phalanges. Deep to these soft tissues, the metatarsals and phalanges provide a clear osseous scaffold for tendon tracking, with dorsal ligaments spanning adjacent joints. Lateral placement is the key landmark. This dorsal relationship matters because extensor digitorum brevis is a common reference point during dorsal approaches to the midfoot and forefoot, where the dorsalis pedis artery and the deep fibular (peroneal) nerve lie in close proximity between extensor hallucis longus and extensor digitorum longus tendons. Surgical exposure for tarsometatarsal fixation, dorsal ganglion excision, or arthrotomy at the metatarsophalangeal joints often requires controlled dissection through thin dorsal skin, and tendon identification prevents iatrogenic injury. Painful dorsal foot swelling can also reflect extensor tendon tenosynovitis or local trauma, and this view clarifies why symptoms localize over the dorsolateral calcaneus and metatarsals.
