Description
Arising from the superolateral surface of the calcaneus, the extensor digitorum brevis forms a fleshy muscle belly on the dorsolateral foot, just anterior to the calcaneus and inferior to the talus at the level of the subtalar joint. From this lateral angle, its tendinous slips course distally and anteriorly toward the lesser toes, lying deep to the extensor digitorum longus tendons as they cross the dorsum over the metatarsals and metatarsophalangeal joints. The lateral column is readable in profile, with the calcaneus posteriorly, the talus superiorly, and the metatarsals projecting distally to the phalanges, while articular cartilage is indicated at the tarsal and toe joints. Clinically, this view clarifies a frequent point of confusion in dorsal foot anatomy: the extensor digitorum brevis is an intrinsic extensor that can be mistaken for tendon swelling or a ganglion along the dorsolateral midfoot. Palpation and local tenderness over the muscle belly, close to the sinus tarsi and anterior process of the calcaneus, can accompany lateral ankle sprains or subtalar joint irritation, and the muscle is also used as a landmark when tracing the deep fibular (peroneal) nerve and dorsalis pedis artery on the dorsum.
