Extensor Digitorum Brevis Under the Skin, Lateral View

Running across the dorsolateral tarsus, the extensor digitorum brevis (EDB) lies immediately deep to the skin and superficial fascia, its fleshy belly originating from the superolateral calcaneus and the inferior extensor retinaculum. Distally, its slips course anteriorly over the talus and cuboid toward the bases of the second to fourth proximal phalanges, paralleling the long extensor tendons and often separating into extensor hallucis brevis medially and the EDB proper laterally. Bony landmarks framed in profile include the calcaneus posteriorly, talus superiorly, and the lateral column of the midfoot leading into the metatarsals. Orientation is clear. Because the EDB is one of the few intrinsic muscles visible on the dorsum of the foot, it provides a reliable surface landmark for the deep fibular (peroneal) nerve and dorsalis pedis artery as they cross the ankle and enter the dorsum between extensor hallucis longus and extensor digitorum longus tendons. Pain and swelling in this region can reflect EDB strain, dorsal midfoot synovitis, or sinus tarsi pathology, and postoperative anatomy matters during dorsal approaches for talonavicular and calcaneocuboid procedures. The muscle belly is also harvested as an EDB flap for small dorsal foot and ankle defects, so depicting its origin and distal tendinous spread supports operative planning and teaching.
