Description
Arising from the superolateral dorsum of the calcaneus, the extensor digitorum brevis forms a low muscular belly that lies anterior to the calcaneal tuber and inferior to the ankle joint line, its short fibers running obliquely anterior and medial toward the dorsal midfoot. Laterally, it sits deep to the extensor retinacula and adjacent to the tendons of extensor digitorum longus as they course across the talus and onto the dorsum, with extensor hallucis brevis typically visible as the medial slip heading toward the proximal phalanx of the hallux. Bony landmarks that frame this relationship include the distal fibula (lateral malleolus), talus, calcaneus, and the cuboid, with dorsal ligaments and the ankle mortise providing positional context. The dorsalis pedis artery and the deep fibular (peroneal) nerve would be expected on the dorsum, with the nerve’s lateral terminal branch supplying extensor digitorum brevis. A lateral view matters because it clarifies how extensor digitorum brevis contributes to metatarsophalangeal extension by adding distinct tendinous slips to digits 2 through 4 while sharing the dorsal tendon corridor with extensor digitorum longus. Surgeons and sports clinicians also recognize this region as a common site for dorsal midfoot pain after inversion injury, where edema or hematoma can mimic an extensor tendon strain, and where the deep fibular nerve may be irritated under the extensor retinaculum.
