Description
Across the dorsum of the male foot, the extensor digitorum brevis (EDB) sits superficial to the tarsal bones, arising from the superior and lateral surface of the calcaneus and adjacent extensor retinaculum before coursing anteriorly toward the lesser toes. Laterally, the extensor hallucis brevis (often considered the medial portion of EDB) angles toward the proximal phalanx of the hallux, while the EDB belly lies lateral to the tendon of extensor digitorum longus and dorsal to the talus, calcaneus, and cuboid. Distally, the muscle divides into slips that merge with the extensor expansions over digits 2 to 4, crossing the metatarsophalangeal joints on the dorsal aspect. Palpation and identification of EDB in this dorsal perspective matters in lateral ankle and dorsal midfoot pain, where swelling can obscure tendon contours and confuse examination findings. The muscle also serves as a practical landmark for the course of the deep fibular (peroneal) nerve and dorsalis pedis artery as they traverse the dorsum of the foot, a relationship that becomes clinically relevant in anterior tarsal tunnel syndrome and during dorsal incisions for midfoot or metatarsal fixation.
