Description
Along the anterolateral compartment of the leg, the extensor digitorum longus is rendered as a long, tapering muscle belly that transitions into multiple tendons crossing the anterior aspect of the ankle toward the dorsum of the foot. Its tendons pass superficial to the tarsal bones and continue distally over the metatarsals to the lateral four toes, where they spread across the dorsal phalanges as extensor expansions. Lateral to the tibial shaft, the fibula and its distal lateral malleolus frame the muscle’s origin and course, while the tarsals, metatarsals, and phalanges provide clear bony reference points for tendon position. Joint spaces are delineated, clarifying the relationships at the tibiotalar and midfoot articulations. Clinical relevance concentrates at the anterior ankle, where the extensor digitorum longus tendons traverse the extensor retinacula and are vulnerable to tenosynovitis from overuse, tight lacing, or anterior ankle impingement in runners. Pain on resisted toe extension and dorsiflexion often localizes along the tendon paths on the dorsum of the foot, and this lateral-sided dorsal tendon anatomy helps distinguish EDL symptoms from extensor hallucis longus or tibialis anterior pathology. The anterolateral trajectory also serves as a landmark when discussing deep fibular (peroneal) nerve territory and compartment anatomy in acute exertional compartment syndrome.
