Description
Viewed from a superior orientation over the dorsum of the foot, the extensor digitorum longus tendon divides into four slips coursing distally toward digits II to V, lying superficial to the tarsals and metatarsals before expanding into the dorsal digital apparatus over the proximal phalanges. Proximally, its tendon crosses the anterior aspect of the ankle joint and tracks centrally across the talus and navicular region, with the slips fanning laterally more than medially as they approach the lesser toes. The tarsals, metatarsals, and phalanges form the bony bed beneath these tendinous bands. Clean lines. Clear relationships. Dorsal tendon topography matters when you are teaching ankle dorsiflexion and toe extension as coupled motions, but it also matters in the clinic when a patient presents with a foot drop pattern and you need to parse tibialis anterior function from extensor digitorum longus recruitment during gait. This superior view helps clarify why irritation beneath the extensor retinacula can produce dorsal ankle pain with resisted toe extension, and why extensor tendon lacerations across the dorsum can selectively disrupt extension of the lesser toes while sparing hallucis extension.
