Description

Running along the anterolateral tibia and fibula, the extensor digitorum longus (EDL) forms a fusiform muscle belly that becomes tendinous distally as it approaches the ankle. From a lateral perspective, its anterior border sits just lateral to tibialis anterior and medial to the fibularis (peroneus) longus and brevis, while extensor hallucis longus lies deeper and slightly medial in the anterior compartment. Distal to the leg, the EDL tendons pass deep to the superior and inferior extensor retinacula, then course on the dorsum of the foot toward digits 2 through 5, lateral to the tendon of extensor hallucis longus. Clear layering. Functionally, EDL dorsiflexes the ankle and extends the lateral four toes, so its line of pull and tendon routing matter when teaching gait and toe clearance in swing phase. Weakness of EDL contributes to foot drop after common fibular (peroneal) nerve injury at the fibular neck, and tenderness or pain along the anterolateral shin often points clinicians toward anterior compartment pathology rather than the posterior calf. The lateral view also clarifies why anterior ankle lacerations and portal placement for ankle arthroscopy must respect the extensor tendons and their retinacular constraints, which can tether the tendons and alter excursion after injury or scarring.
