Description

Running along the anterolateral compartment of the male leg, the extensor digitorum longus (EDL) occupies the lateral side of tibialis anterior and lies anterior to the fibula, with its muscle belly tapering distally into multiple tendons. Near the ankle, the EDL tendons pass anterior to the talocrural joint and are held close to the skeleton by the extensor retinacula before diverging onto the dorsum of the foot toward the lateral four digits. Peroneus tertius often appears as a distal lateral slip blending with EDL and inserting on the base of the fifth metatarsal, a small but helpful landmark for orientation. Bone contours of the distal tibia, fibula, and the lateral malleolus set the lateral boundary of this corridor. Lateral anatomy of EDL matters because it clarifies how dorsiflexors and toe extensors share the tight anterior compartment and how their tendons negotiate the ankle without bowstringing, a relationship that becomes obvious when considering anterior compartment syndrome, traumatic swelling, or post-fracture scarring. Weakness of EDL contributes to steppage gait and impaired toe extension in deep fibular (peroneal) nerve injury or L5 radiculopathy, and this view helps distinguish EDL dysfunction from isolated tibialis anterior weakness during bedside motor testing. The peroneus tertius slip also has clinical consequences. It can be involved in traction pain at the dorsolateral foot and is sometimes used as a reference in surgical approaches to the anterior ankle.
