Description
Running along the anterolateral compartment of the leg, the extensor digitorum longus muscle tapers into multiple tendons that pass anterior to the ankle joint beneath the extensor retinacula, then fan distally toward digits two through five on the dorsum of the foot. Lateral to this tendon group, the fibularis (peroneus) longus and fibularis brevis course posterior to the lateral malleolus within the common fibular tendon sheath, separated from the anterior extensor tendons by the fibula and the lateral malleolar contour. Superficially placed skin is rendered translucent so you can appreciate the tendons’ subcutaneous trajectory as they cross the tarsus, with neighboring vessels and selected tarsal and distal tibiofibular landmarks providing orientation. Clean tendon geography. This lateral perspective matters when teaching the functional “crossing” at the ankle, where dorsiflexors (including extensor digitorum longus) run anterior to the talocrural axis while the fibularis tendons run posteriorly and act as evertors. Clinically, it supports discussion of anterior ankle and dorsal foot tendonitis, extensor retinaculum related tenosynovitis, and iatrogenic risk during anterolateral ankle arthroscopy or portal placement where the superficial fibular nerve and extensor tendons can be vulnerable. The relationship to the lateral malleolus also helps clarify why fibularis brevis tears and peroneal tendon subluxation present posterolaterally, not on the dorsum.
