Description
From a dorsal perspective of the male foot, the extensor digitorum longus tendons emerge anterior to the ankle joint and course distally beneath the superior and inferior extensor retinacula, then fan laterally across the dorsum to the lateral four digits. Each tendon divides into an extensor expansion, with a central slip to the base of the middle phalanx and paired lateral bands continuing to the base of the distal phalanx of toes 2 to 5. Medial to these slips you would expect the extensor hallucis longus tendon heading toward the hallux, while laterally the fibularis (peroneus) tertius tendon often blends near the base of the fifth metatarsal. This dorsal tendon arrangement matters when you are teaching or treating toe extension mechanics and common dorsal foot pain patterns. Overuse tenosynovitis of the extensor tendons, irritation under a tight shoe tongue, and hypertrophy or thickening of the extensor retinacula can all reproduce pain over the anterior ankle and dorsum, and this view clarifies why symptoms often localize between the tendons during resisted extension of toes 2 to 5. It also supports surgical and procedural anatomy: dorsal approaches to the midfoot and metatarsophalangeal joints must respect these tendon trajectories, and local anesthetic blocks or ultrasound-guided injections are planned around the extensor compartments.
