Description
Running anteroinferiorly along the lateral aspect of the distal leg, the extensor digitorum longus (EDL) tendon emerges from the anterior compartment and divides into four slender slips that course toward digits II to V. At the level of the ankle, these tendons pass deep to the superior and inferior extensor retinacula, lying anterior to the talocrural joint and lateral to the tibialis anterior tendon, with the extensor hallucis longus tendon positioned between them. Fibularis (peroneus) longus and brevis tendons track posterior to the lateral malleolus, creating a clear lateral contrast between the extensor group anteriorly and the peroneal tendons posteriorly. Dorsal arteries and veins accompany the tendons across the dorsum of the foot, superficial to the tarsals, metatarsals, and phalanges. A lateral perspective makes tendon routing and retinacular constraint easy to teach, since you can follow how EDL changes direction at the ankle before fanning distally to the toes. Clinically, this matters in anterior ankle tendinopathy and extensor tenosynovitis, where thickening within the extensor tendon sheath or beneath the inferior extensor retinaculum produces pain with resisted toe extension and may mimic an ankle sprain. It also supports surgical planning for dorsolateral approaches to the ankle and midfoot, where respecting the extensor tendons and the superficial dorsal venous network reduces postoperative adhesions and wound problems.
