Beneath the dorsal skin of the foot, the extensor digitorum longus tendons course anteriorly over the ankle and spread into four slips toward the lateral four toes, lying superficial to the metatarsals and phalanges and tracking distally toward the dorsal digital expansions. Laterally, the fibularis (peroneus) tendons skirt the lateral malleolar region and head toward their insertions on the midfoot, contrasting with the more central extensor tendons that run in parallel bands across the dorsum. Fine superficial veins and small cutaneous nerves cross the tendons obliquely, while deeper dorsal vessels align closer to the intermetatarsal spaces. Bony landmarks stay readable through the soft tissue planes. A dorsal, subcutaneous perspective matters because this is the working corridor for common procedures and injuries: extensor tendon lacerations from dorsal foot trauma, tendon adhesions after fracture fixation, and iatrogenic injury during dorsal incisions for metatarsophalangeal or midfoot exposure.
