Description
Layered beneath the skin on the lateral aspect of the male lower leg, the extensor digitorum longus occupies the anterior compartment, positioned lateral to tibialis anterior and anteromedial to the fibularis (peroneus) longus and brevis along the fibular shaft. Its muscle belly tracks inferiorly toward the ankle, where the extensor tendons continue distally onto the dorsum of the foot en route to digits two through five. Posteriorly, the gastrocnemius overlies the soleus to form the calf mass, creating a clear posterior compartment contrast against the more anterior, cordlike extensor tendons. Tibia sits medial and broad, while the fibula remains the slender lateral strut and a key landmark for compartment boundaries. For teaching and clinical correlation, the lateral relationship between extensor digitorum longus and the fibula helps orient you during assessment of anterior compartment problems, where pain with resisted toe extension and dorsiflexion can accompany exertional compartment syndrome or tendinopathy of the extensor tendons under the extensor retinacula. Proximity matters in trauma: fibular shaft fractures and associated swelling can distort the local anatomy, and injury patterns near the fibular head raise concern for common fibular (peroneal) nerve involvement, which can present as foot drop and altered dorsum foot sensation.
