Extensor Hallucis Brevis Beneath the Skin, Dorsal View

Description

Arising from the superolateral calcaneus, the extensor hallucis brevis (EHB, extensor hallucis brevis pedis) lies on the dorsal aspect of the foot deep to the skin, immediately medial to the extensor digitorum brevis. Its short muscle belly transitions into a tendon that courses anteriorly and distally to the base of the proximal phalanx of the hallux, crossing the dorsal surface of the first metatarsophalangeal joint. Surrounding bony landmarks include the calcaneus and other tarsals proximally, the metatarsals in the midfoot, and the phalanges distally, with the EHB tendon sitting superficial to the dorsal joint capsules and periosteum. Functionally, EHB augments extension of the great toe at the metatarsophalangeal joint and is often discussed alongside extensor hallucis longus when teaching hallux mechanics during gait. Because the muscle belly sits in the dorsolateral midfoot, this perspective also supports clinical discussions of dorsal midfoot pain, where local trauma, postoperative scarring, or compartment swelling can involve the extensor digitorum brevis and adjacent EHB.
