Description
Oriented in dorsal (superior) aspect, the foot is rendered with the extensor compartment and intrinsic dorsal musculature layered over the tarsals, metatarsals, and phalanges. Extensor hallucis longus and extensor digitorum longus tendons course distally across the dorsum, crossing the ankle region and spreading toward the hallux and lateral four digits, while extensor hallucis brevis and extensor digitorum brevis sit more proximally on the dorsolateral midfoot. Between the metatarsals, the dorsal interossei are visible as slender muscle bellies occupying the intermetatarsal spaces, positioned deep to the long extensor tendons but superficial to the metatarsal shafts. Bony landmarks such as the talus, calcaneus, navicular, cuboid, and cuneiforms provide the posterior-to-anterior scaffold for the dorsal soft tissues. Dorsal anatomy matters because it is where thin tendon sheaths, dorsal capsule reflections, and superficial neurovascular structures are most vulnerable to traction and compression. A clear dorsal map helps when discussing extensor tendonitis and tenosynovitis, and when localizing pain in anterior ankle impingement or midfoot sprain patterns that involve the tarsometatarsal (Lisfranc) region.
