Description
Cut in a lateral plane, the section opens the cardiac chambers to show the thick left ventricular myocardium lateral to the interventricular septum, with the left atrium positioned superior and posterior to the ventricle. Valve leaflets at the atrioventricular junction are supported by chordae tendineae that course inferiorly to papillary muscles projecting from the ventricular wall. Externally, epicardial fat and segments of the coronary vessels lie on the surface, with red and blue coding separating arterial and venous pathways along the atrioventricular and interventricular grooves. Small structures, big consequences. Chordae tendineae and papillary muscles are the mechanical safeguard against atrioventricular valve prolapse during systole, so this lateral cross section is well suited for explaining why leaflet coaptation fails in acute papillary muscle rupture after myocardial infarction, producing abrupt severe mitral regurgitation. The cut also helps distinguish true leaflet pathology from annular dilation by orienting the viewer to the fibrous skeleton of the heart at the valve ring, where the leaflets hinge and where calcification can stiffen the orifice. For teaching, it clarifies how ventricular geometry and tethering forces translate into functional regurgitation even when the leaflets are structurally normal.
