Description
Centered in an anterior thoracoabdominal view, the male heart rests on the tendinous center of the diaphragm, with the right and left hemidiaphragms doming superiorly beneath it. Superiorly at the cardiac base, the ascending aorta lies posterior and slightly rightward relative to the pulmonary trunk, while the right and left pulmonary arteries course laterally from the pulmonary trunk; the superior vena cava descends on the right side of the mediastinum to enter the right atrium. The right atrium and right ventricle form most of the anterior cardiac silhouette, with the left ventricle contributing more inferolaterally toward the apex. Seeing the great vessel roots from the front matters because this is the orientation encountered in median sternotomy and in many teaching diagrams that introduce mediastinal anatomy. The spatial relationship between the pulmonary trunk and ascending aorta clarifies why pulmonary artery catheterization tracks anteriorly before passing into the right and left pulmonary arteries, and why pericardial reflection around the arterial trunks defines the transverse pericardial sinus. Small shifts in diaphragmatic position also change the apparent cardiac axis. This is the baseline for interpreting elevation of the hemidiaphragm, phrenic nerve palsy, and diaphragmatic eventration with secondary effects on cardiac position. Use this illustration in gross anatomy and cardiothoracic surgery modules to teach the anterior surface of the heart, the base and great arteries, and the functional coupling of diaphragm and pericardium during respiration.
