Description
Seen from the lateral aspect of an adult male pelvis, the acetabulum of the coxal bone is rendered with prominent osteophytes along the acetabular rim, projecting outward from the superior and anterior margins where the labrum normally attaches. Inferiorly, the proximal femur aligns with the socket, and the native femoral head has been replaced by a smooth spherical femoral component that sits within an acetabular cup fixed to the pelvic bone. The ilium lies superior to the joint line, while the ischium and pubis form the posteroinferior and anteroinferior walls of the acetabulum, framing the prosthetic articulation in profile. Bone texture is intentionally porous and trabecular, making the rim outgrowths easy to read against the surrounding cortical surface. Lateral views are where acetabular rim spurs matter. Osteophytes at the anterosuperior rim are a common morphologic substrate for pincer-type femoroacetabular impingement and often coexist with degenerative osteoarthritis that ultimately leads to total hip arthroplasty; if not recognized and addressed, residual rim osteophytes can contribute to reduced range of motion, iliopsoas irritation over the anterior cup, and bony impingement that increases the risk of levering the prosthetic head toward dislocation. The reddish periacetabular tissue cue fits typical teaching about synovitis, capsular irritation, or reactive change around the cup-bone interface. A practical surgical landmark.
