Description
Posterior hip anatomy centers on the coxal bone and proximal femur, with the femoral head seated in the acetabulum and the femoral neck projecting inferolaterally toward the greater trochanter. Along the posterior acetabular rim and adjacent femoral head neck junction, osteophytes (bone spurs) protrude into the periarticular space, a pattern that often extends toward the ischial region near the lesser trochanter. Medially, the ischium forms the posterior inferior pelvis, and the ischiofemoral interval lies between the ischial tuberosity and the posteromedial proximal femur. Inflamed periarticular tissue is suggested by the red perifocal coloration around the joint margins. Clear pathology. Posterior osteophytes matter because they are common in hip osteoarthritis and can be underestimated on standard anteroposterior pelvic views, yet they restrict internal rotation and extension and can contribute to mechanical conflict in the ischiofemoral region where the quadratus femoris traverses between the ischium and femur. Hypertrophic spurring and reduced ischiofemoral space correlate with ischiofemoral impingement and deep gluteal pain patterns, and they also influence posterior surgical planning, including the risk of impingement after total hip arthroplasty if component version does not account for posterior bony overgrowth. The posterior margin is a surgical reality.
