Description
Viewed from a lateral perspective, the anterior compartment of the male leg is bounded medially by the lateral surface of the tibia and laterally by the anterior intermuscular septum adjacent to the fibula, with deep crural fascia enclosing the compartment. Tibialis anterior occupies the anteromedial plane, while extensor hallucis longus lies deep and slightly lateral, and extensor digitorum longus tracks along the anterolateral aspect toward the dorsum of the foot. Distally, their tendons converge toward the anterior ankle beneath the superior and inferior extensor retinacula, continuing to insert on the medial cuneiform and first metatarsal (tibialis anterior), distal phalanx of the hallux (extensor hallucis longus), and the dorsal expansions of digits two through five (extensor digitorum longus). Clear compartmental boundaries. This lateral angle matters because it clarifies where the anterior tibial artery and deep fibular (peroneal) nerve course on the interosseous membrane before running between tibialis anterior and extensor digitorum longus, a relationship that is tested in anterior compartment syndrome and in distal leg trauma. The same anatomy frames common entrapment patterns, including deep fibular nerve irritation under the extensor retinaculum and functional weakness of ankle dorsiflexion leading to foot drop. Surgeons also rely on these planes when planning an anterolateral approach to the ankle or when placing incisions that respect the intermuscular septa and neurovascular bundle.
