Description
Seen from the lateral temporal region, the auricle (pinna) sits superficial to the lateral skull, with the external acoustic meatus directed medially into the temporal bone. Overlying the cartilaginous framework are the extrinsic auricular muscles, including auricularis anterior and auricularis superior anterior to and superior to the root of the helix, and auricularis posterior positioned posterior to the auricle near the mastoid region. Fine intrinsic auricular muscles track along the cartilage contours of the helix, antihelix, tragus, and concha, just deep to the skin. Skin texture and follicles remain intact as a superficial layer, while hints of the temporoparietal fascia and adjacent facial musculature can be inferred around the superior and anterior margins. Functionally, this view clarifies how the auricular muscles relate to the superficial temporal area and the periauricular fascia planes that surgeons and proceduralists traverse. It is the anatomy you want in mind when planning a preauricular incision, raising a temporoparietal fascial flap for auricular reconstruction, or placing periauricular local anesthetic while respecting nearby sensory supply from the auriculotemporal nerve and great auricular nerve. Small structures, small mistakes. Even subtle muscle position helps orient the course of superficial vessels such as the superficial temporal artery as it ascends anterior to the ear.
