Description
Running along the posterolateral trunk, the iliocostalis lumborum forms the lateral column of the erector spinae, positioned lateral to longissimus and separated from multifidus by the thoracolumbar fascia. From a lateral perspective, its fleshy and tendinous slips arise inferiorly from the iliac crest and the common erector spinae aponeurosis near the posterior iliac spine, then ascend superiorly to the angles of the lower ribs. Adjacent bony landmarks include the lumbar vertebrae with intervertebral discs, the inferior rib cage, and the iliac wing. Clear relationships. Clinically, this view helps localize posterior lumbar pain generators and the surface anatomy you palpate in the posterior axillary line, where iliocostalis lumborum often develops myofascial trigger points that refer pain toward the flank and iliac crest.
