Description
Running longitudinally along the posterolateral lumbar spine, the iliocostalis lumborum forms the lateral column of the erector spinae, positioned lateral to longissimus thoracis and superficial to the lumbar vertebrae and thoracolumbar fascia. Its thick inferior attachment arises from the posterior iliac crest and adjacent sacrum, then ascends to insert onto the angles of the lower ribs and the transverse processes of the upper lumbar vertebrae. From an external view of the male torso, you would also expect to see the spinous processes in the midline, the posterior ribs superiorly, and the iliac crests framing the muscle’s broad, tendinous base. It is a lateral stabilizer. Clinically, this muscle matters because it becomes a palpable landmark in mechanical low back pain and paraspinal strain, where localized tenderness often sits just lateral to the spinous processes rather than in the midline. Its fascial continuity with the thoracolumbar fascia links iliocostalis lumborum to load transfer between the rib cage and pelvis, a concept used when teaching lumbar extension and lateral flexion mechanics, and when explaining why repetitive lifting and sustained trunk rotation can provoke symptoms. Differentiating iliocostalis from psoas major is also a common teaching point, since psoas is an anterior hip flexor, while iliocostalis is posterior and acts on the vertebral column and ribs.
