Description
Posteriorly, the iliocostalis lumborum is isolated as the lateral column of the erector spinae, lying lateral to longissimus thoracis and separated from the midline spinous processes by the thoracolumbar fascia. Inferiorly it anchors along the iliac crest and posterior sacral region, then ascends superolaterally as a series of slips to the angles of the lower ribs, classically ribs 6 through 12. Rib contours and the posterior elements of the thoracolumbar spine provide bony reference for the staggered, segmental insertions. Orientation is unambiguous. Emphasis on iliocostalis lumborum matters because it is a frequent pain generator at the thoracolumbar junction, where traction on the rib angles and load transfer through the iliac crest can irritate the adjacent thoracolumbar fascia. Palpation and dry needling targets often follow the rib angles lateral to the paraspinal gutter, so seeing the muscle’s lateral position relative to the spine helps avoid drifting medially toward multifidus and the dorsal rami.
