Description
Seen from the lateral aspect, a short vertebral segment is rendered with stacked vertebral bodies anteriorly and the spinous processes projecting posteriorly, separated by thin, membranous interspinous ligaments spanning between adjacent spinous processes. The supraspinous ligament forms a more continuous band along the apices of the spinous processes, positioned posterior to the interspinous sheets. Intervertebral discs sit between vertebral bodies, while the posterior elements (laminae and articular processes) frame the posterior margin of the spinal canal. Paraspinal muscle bundles run longitudinally along the posterior column. Clear relationships. Clinically, the interspinous and supraspinous ligaments are key restraints to flexion, and their failure contributes to posterior ligamentous complex injury patterns in thoracolumbar trauma and degenerative instability. A lateral perspective also maps directly onto the needle path for midline and paramedian lumbar puncture or neuraxial anesthesia, where the supraspinous ligament is encountered posteriorly before the interspinous ligament and ligamentum flavum. Posterior interspinous widening on flexion radiographs and focal edema on MRI can indicate sprain or disruption at a specific motion segment.
