Interspinous Ligament, Posterior View

Description

Running in the midline between adjacent spinous processes, the interspinous ligament spans the interspinous spaces from the root to the apex of each process, blending posteriorly with the supraspinous ligament. A posterior perspective also implies the dorsal elements of the vertebrae, including stacked spinous processes and the laminae just lateral to them, with paired intrinsic back muscles lying on either side. Medial fibers sit deep to the superficial fascial plane, while the paraspinal muscle masses remain posterolateral to the ligamentous complex. Clear midline anatomy. Functionally, this ligamentous band is one of the posterior stabilizers that resists flexion, working with the supraspinous ligament and ligamenta flava as part of the posterior tension band of the spine. Clinically, it becomes a landmark in neuraxial procedures: during a midline lumbar puncture or epidural, the needle traverses supraspinous ligament then interspinous ligament before contacting ligamentum flavum, and loss of resistance is interpreted in relation to these layers. Degenerative change or widening in the interspinous interval, as in Baastrup disease (kissing spines), can also be discussed with this view because the pathology centers on adjacent spinous processes and their intervening soft tissues. Anatomy faculty commonly pair this posterior view with surface palpation teaching of lumbar spinous processes and with lab instruction on posterior spinal ligaments in gross anatomy and orthopaedic modules. It also fits well in anesthesiology teaching files, procedure manuals, and patient education graphics explaining the tissue planes encountered in epidural access.
