Description
Posteriorly, the distal tibia and fibula frame the ankle mortise while the posterior tibiofibular ligament spans the back of the syndesmosis as a stout, obliquely oriented band from the posterior tibial tubercle to the posterior aspect of the lateral malleolus. Deep to the more superficial fibers, the transverse tibiofibular ligament forms a thickened inferior component that sits immediately superior to the posterior trochlear margin of the talus, tightening the posterior rim of the mortise. The calcaneus lies inferior and posterior, with the Achilles tendon coursing centrally toward its insertion on the calcaneal tuberosity. Attention to this posterior perspective matters because the posterior inferior tibiofibular ligament complex is the primary restraint to posterior translation and external rotation at the distal tibiofibular joint, and it is commonly injured in high ankle sprains that also disrupt the anterior inferior tibiofibular ligament and interosseous membrane. Arthroscopically and on MRI, these deep transverse fibers can be mistaken for posterior capsular tissue, yet their disruption changes talar positioning and can leave the mortise subtly widened under stress. Small structure, big consequences.
