Description
Running inferior to the lateral malleolus, the inferior fibular (peroneal) retinaculum forms a thickened band of crural fascia that tethers to the lateral surface of the calcaneus while spanning over the fibularis longus and fibularis brevis tendons as they curve posterior to the distal fibula. A lateral, posterior-leaning perspective places the Achilles tendon on the posterior heel, with the calcaneus deep to it and the talus seated superior to the calcaneus at the ankle mortise. Distal to the hindfoot, the lateral column continues into the cuboid and metatarsals, providing landmarks for the peroneal tendon course along the lateral foot. Tight anatomy. Clear relationships. This retinaculum matters because it functions as the primary restraint preventing peroneal tendon bowstringing and subluxation as the tendons change direction around the lateral malleolus, a common pain generator after inversion ankle sprain. Clinically, tears or laxity of the peroneal retinacula can accompany peroneal tendon split tears, chronic lateral ankle instability, or a shallow fibular groove, and surgical repair often references the calcaneal attachment and the retromalleolar groove as intraoperative landmarks. The posterior-lateral view also supports teaching the distinction between retinacular pathology and lateral ligament injury (anterior talofibular and calcaneofibular ligaments) in the differential for persistent lateral ankle symptoms.
