Description
Oriented to the posterior ankle, the illustration centers on the posterior talofibular ligament spanning from the malleolar fossa on the medial surface of the lateral malleolus (distal fibula) to the posterolateral talus, adjacent to the lateral tubercle of the talar posterior process. The distal tibia and fibula frame the talar trochlea superiorly, while the subtalar region and proximal calcaneus sit inferior to the talus, partially obscured by the posterior capsuloligamentous complex. Lateral to the posterior talofibular ligament, the calcaneofibular ligament is suggested as a more inferior band descending toward the calcaneus, and a thin posterior joint capsule blends medially with fibers near the posterior intermalleolar ligament. Clear bony landmarks anchor the orientation. Posterior talofibular ligament anatomy matters because it behaves differently from the anterior talofibular ligament in lateral ankle sprains: it is thick and often spared in routine inversion injury, but it becomes relevant in high-grade instability, subtalar involvement, and posterior ankle trauma where the posterior talus or lateral malleolus is affected. This posterior perspective helps when teaching why a talar posterior process fracture (including the lateral tubercle, sometimes called Shepherd fracture) can mimic a “sprain,” and how posterior capsular scarring or impingement may limit plantarflexion. Strong attachment at the malleolar fossa is the point.
