Description
From a lateral ankle perspective, the distal tibia and fibula frame the talar trochlea, with the posterior inferior tibiofibular ligament spanning obliquely between the posterior tibial tubercle and the posterior aspect of the lateral malleolus. Deep fibers lie posterior to the distal tibiofibular joint line and blend with the posterior syndesmotic capsule, while the fibula remains lateral to the tibial incisura. Inferiorly, the talus sits beneath the tibial plafond, and the calcaneus and lateral column tarsals extend distal to the hindfoot. Tendons crossing the lateral ankle, including the fibularis (peroneus) longus and brevis posterior to the lateral malleolus, provide a useful topographic reference. This ligament matters because it is the stout posterior brace of the syndesmosis, resisting posterior translation and external rotation of the fibula relative to the tibia when the ankle is loaded in dorsiflexion. Miss it, and you miss syndesmotic stability. In high ankle sprains, disruption of the posterior inferior tibiofibular ligament and its deep component (often described clinically as the transverse tibiofibular ligament) widens the mortise and can produce persistent pain and malreduction after fibular fixation if the syndesmosis is not anatomically restored.
