Description
Seen from the lateral side of the male ankle, the distal fibula (lateral malleolus) sits superior and lateral to the talus, with the calcaneus positioned inferior and posterior as the hindfoot base. The posterior talofibular ligament (PTFL) spans from the malleolar fossa on the posterior aspect of the fibula to the posterolateral talus, running predominantly horizontal and deep within the posterior ankle recess. Adjacent lateral collateral components are implied in this corridor, with the calcaneofibular ligament coursing more inferiorly toward the calcaneus while the talocrural joint line remains centered between tibial plafond and trochlea tali. Bony landmarks of the midfoot and forefoot extend distally, providing orientation for hindfoot alignment. PTFL anatomy matters because it is the stoutest element of the lateral ligament complex and is typically stressed in severe inversion injuries, often alongside disruption of the anterior talofibular ligament and calcaneofibular ligament.
