Description
Seen in lateral profile, the distal tibia and fibula form the ankle mortise superior to the talus, with the posterior tibiofibular ligament spanning between the posterior tibial malleolus and the posterior aspect of the distal fibula at the lateral malleolus. Inferiorly, the talus articulates with the calcaneus, and the calcaneal tuberosity and lateral calcaneal wall provide bony landmarks posterior to the subtalar joint. Anterior to the hindfoot, the lateral column of the foot continues through the cuboid to the bases and shafts of the fourth and fifth metatarsals, with the phalanges extending distally. That posterior tibiofibular complex matters because it is the primary restraint to posterior translation and external rotation of the fibula at the distal tibiofibular syndesmosis, and it tension-links the mortise during dorsiflexion when the wider anterior trochlea of the talus seats into the joint. Syndesmotic sprain patterns often involve the posterior inferior tibiofibular ligament and its deep component, the transverse tibiofibular ligament, and failure here widens the mortise even when the anterior talofibular ligament remains intact. This is the lateral view surgeons want when thinking about posterior malleolar fractures and fixation strategy, where the posterior syndesmosis frequently rides with the posterior malleolar fragment.
