Description
Posteriorly, the supraspinous ligament forms a continuous fibrous band along the apices of the spinous processes from the prominent C7 spinous process through the thoracic and lumbar vertebrae to the sacrum, lying in the midline superficial to the interspinous ligaments. Superiorly it blends with the ligamentum nuchae, which fans cranially toward the external occipital crest and nuchal lines, creating a palpable longitudinal landmark between the paired paraspinal muscle masses. Deep to this band sit the spinous processes and laminae, while laterally the ribs articulate with the thoracic vertebrae and the posterior iliac crests frame the caudal extent of the column. Midline anatomy is the point. Because the supraspinous ligament and ligamentum nuchae limit flexion of the cervical, thoracic, and lumbar spine, a clean posterior perspective helps clarify how midline tension is transmitted across multiple motion segments rather than isolated to one interspinous space. Clinically, this anatomy is referenced when grading posterior ligamentous complex integrity in thoracolumbar trauma, and in inflammatory conditions such as ankylosing spondylitis where enthesitis and subsequent ossification along the midline can contribute to the classic stiff, kyphotic spine. It also grounds teaching of posterior surgical landmarks, where the spinous processes guide level identification and midline approaches before deeper dissection reaches lamina and facet joints.
